Status shows Disconnected

Usually the refresh token died or someone revoked the app. Try Reconnect or Connect New File. If it keeps happening, check Xero's connected apps and whether your user still belongs to that organisation.

Accounts or invoices will not load

Confirm the connection first. New password or MFA at Xero often needs a fresh link. If the API returns auth errors, make sure the app is still approved for that org in Xero.

Wrong organisation

Switch using Current Entity in the header or choose Return to Entity List to pick another file from Connect. Disconnect files you no longer support so the menu stays short.

Figures look old

On Connect, press Refresh Status. Inside the app, use any refresh or sync control on the screen you are on. Xero can also lag slightly in edge cases.

Posted the same period twice

Fix it in Xero first (reverse or void duplicate manual journals per your policy). Then tighten who posts and whether you use a checklist so the same period is not sent twice.
